Provide one valid, unexpired, government-issued identification document that includes your photograph and signature.

- A Permanent Resident Card, commonly called a Green Card
- A U.S. military identification card
- A Connecticut state pistol permit
Secondary Identification: One Additional Form Required
The second form of identification must include your signature, but it does not need to include a photograph.

- A work or employee identification badge that includes your signature
- A signed library card
- Another current identification document issued by a government agency, institution, or business that includes your signature
