Prepare for Your Mobile Notary Appointment

I'll come to your home, office, hospital, nursing facility, assisted living community, or another mutually agreed-upon location. To help your appointment go smoothly, please have your documents and identification ready before I arrive.

Connecticut law requires each signer to present two forms of identification, including one valid government-issued photo ID that includes both a photograph and signature. Please have your identification available for verification when I arrive.

Primary Identification (Choose One)

Provide one valid, unexpired, government-issued identification document that includes your photograph and signature.

  • A valid driver's license or state-issued identification card, such as a Connecticut DMV driver's license or non-driver ID
  • A U.S. or foreign passport
  • A Permanent Resident Card, commonly called a Green Card
  • A U.S. military identification card
  • A Connecticut state pistol permit

Secondary Identification: One Additional Form Required

The second form of identification must include your signature, but it does not need to include a photograph.

  • A signed credit or debit card
  • A work or employee identification badge that includes your signature
  • A signed library card
  • Another current identification document issued by a government agency, institution, or business that includes your signature
  • The unsigned document(s) — do not sign before I arrive
  • Any additional signers who need to be present
  • Payment for services (for general notary appointments — loan signing fees are handled separately through the title company or lender)
